📋 Basic Requirements
Original Philippine Passport
- Must be signed
- Must have at least two (2) blank visa pages
Passport Bio Page Copy
- Clear photocopy of the passport information page
Japan Visa Application Form
- Fully completed and signed
One (1) Recent Passport-Size Photo
- White background
- 4.5 cm × 3.5 cm
- Taken within the last six (6) months
PSA Birth Certificate
- Issued by PSA
PSA Marriage Certificate
- Required for married applicants
- Must be PSA-issued

Required if the applicant will personally pay for the trip:
Bank Certificate
Bank Statement
- Last 6 months, if required
Latest Income Tax Return (ITR) or BIR Form 2316
Certificate of Employment
- Must indicate position, salary, and employment duration
For Business Owners:
- Business Registration (DTI/SEC/CDA)
- Mayor's Permit (if applicable)
- Latest Income Tax Return
For Students:
- School ID
- Certificate of Enrollment
For Retirees / Unemployed Applicants:
- Explanation Letter (if requested)
- Financial documents of sponsor
👨👩👧 Sponsored Travel
If another person will shoulder your travel expenses:
- Sponsorship / Guarantee Letter
- Sponsor's Bank Certificate
- Sponsor's Bank Statement
- Sponsor's Certificate of Employment or Business Documents
- Proof of Relationship (Birth Certificate, Marriage Certificate, or other supporting documents)
📄 Additional Documents

- Authorization Letter (if applicable)
- PSA Authorization (if a representative will request PSA documents)
